As a member of the case management program, the medical social worker works in partnership with the nurse case manager and other healthcare professionals to assure continuity ! and coordination of care for patients and families. The medical social worker furnishes services and consultation to patients/families regarding social, environmental, psychological, financial and cultural factors related to illness and hospitalization, supports continuity in case management and provides discharge management.
Qualifications:
1. BSW or equivalent level degree from an accredited school.
2. HS AED CPR must be obtained within first week of employment.
Job Duties:
1. Conducts thorough psychosocial assessment of patients utilizing all available resources.
2. Provides assistance with evaluation and referral in situations of possible abuse/neglect for adult and pediatric patients.
3. Documents assessment findings and social service interventions appropriately in the medical record and communicates same to the appropriate health care team members.
4. Implements social service interventions based on psychosocial! assessment.
5. Assists patients/family members in obtaini! ng Advanced Directives and/or Durable Power of Attorney for Health Care forms.
6. Provides patient and/or family counseling to assist with coping, grieving, conflict resolution, etc.
7. Acts as a resource to patients and family members and the health care team regarding community and governmental resources.
8. Works collaboratively with case managers and the health care team in formulating and facilitating patients’ hospital discharge plans.
9. Acts as a liaison between patients, family members, physicians, interdisciplinary team, and external agencies and organizations.
10. Acts as a resource/advisor to physicians regarding social work issues that may affect patient care.
11. Maintains knowledge and understanding of KRMC policies and procedures, Medicare/Medicaid, managed care and other payer regulations and benefit limits.
12. Develops, facilitates and/or participates in support groups as directed.
13. Develops and maintains professional ! relationships and knowledge of services, resources and providers across the healthcare continuum.
14. Works collaboratively and professionally with patients, family members, physicians, hospital staff and other individuals and agencies involved in providing patient care.
15. Ensures the notice of Medicare rights are given to Medicare patients within 2 days of discharge.
